The NL has 16 teams, and the AL has 14. Otherwise, the only major remaining difference is the American League’s choice to implement the designated hitter rule.
In the past, the two leagues had used different umpires and even different baseballs (though only the markings were different, not the actual ball composition). They had a few different minor rules, though nothing in terms of gameplay – the curfew rule, for example.
Now, the DH is the only difference left.
EDIT: To clarify a subsequent answer, the two leagues use the same rule book, so the strike zone is the same. They also use the same umpires.
